If you want to change your address or contact details, update your beneficaries, provide your TFN or alter your investment options for future contributions (not account balance mix) you can do it now online and save a few trees as well as help keep your fund's costs down.

Employers can also submit contribution data online.

  • Download Change in Details Form TF4.3 (PDF, 155kb)

    STOP - You can change your address details, contact information or provide your TFN yourself, in the secure member log in area. Only use this form to let us know you’ve changed your name or employer.
  • Download Change in Investment Options TF6.1 (PDF, 130kb)

    STOP - If you just want to change your asset mix for future contributions only and not your existing account balance, you can do it now online in the secure member log in area. If you want to change the investment options for your account balance then you need to complete this orm and send it to us.
  • Download Change in Nominated Beneficary Form TF5.1 (PDF, 130kb)

    Use this form to change your nominated beneficaries.
  • Download Change in Insurance Options TF3.1 (PDF, 170kb)

    Use this form to apply for or change your level and or type of insurance cover.

    Use this form to transfer in super money you may have with other super funds. While we don't charge any fees for this service, please check that your other funds won't charge you fees for leaving. It's also wise to confirm any insurance arrangements you might have before closing your other super accounts.

    Download Personal & Self Employed Contribution Remittance Advice Form TF30.3 (PDF 159kb)

    Use this form to make additional personal contributions, spouse contributions or contributions as a self-employed individual.
  • Download Letter of Compliance (PDF, 89kb)

    Use this letter if you wish to transfer in super money you may have with other super funds. This letter may need to be used in conjunction with the Transfer in Authority Form above or with a Choice of Superannuation Fund Form.

    Use this form to select the WALGSP as your fund to which your employer will contribute on your behalf.
  • Download Superannuation Contribution Splitting Form (PDF, 122kb)

    You should complete this application if you want to split some of your super contributions for the benefit of your spouse .
  • Download Advice of Employee Future Contribution Mix Form TF11.1 (PDF, 124kb)

    This form is for employees to advise their employers of a change in the type and or amount of superannuation contribution they wish to have deducted from their pay. Do not send this form to us.
